Industry shifts and regulatory changes, such as the move to a Fee-for-Outcome model, are driving the need for health systems and senior care providers to be more interoperable in an effort to achieve better care coordination across the continuum. With the right data connections, care providers, and their health system partners, will have the right information to make the best care delivery decisions that create the best possible health outcomes.
Close collaboration helps ensure patients are placed into the right care setting after discharge from hospitals, increasing the likelihood of a swift recovery. Additionally, by working together, health systems and LTPAC providers can achieve an appropriate length of stay for each patient, reduce hospital readmissions, and improve care efficiencies.
Empowering health systems, with the visibility required to support a truly coordinated and collaborative approach to care, is no small feat, but will provide healthcare partners with the patient insights required for true coordinated care.
